Vital Records (Birth & Death Certificates)

Tooele County Health Department provides certified copies of birth certificates for births that occurred in Utah from 1931 to the present and death certificates that occurred in Utah after July 2006 (all other dates available at the State Office – see below how to order online).  For marriage records visit the county clerk office where the marriage occurred and for divorce records visit the district court where the divorce was finalized. Order Certificate Options: IN PERSON, BY MAIL, PHONE OR ONLINE

IN PERSON– Order IN PERSON Monday through Thursday from 8:00 a.m. – 6:00 p.m., Friday 8:00 a.m. – noon, closed weekends, and holidays.  All in-person orders require an application, valid identification and correct fees.  Requests will be completed the same day.

BY MAIL OR PHONE- Call (435)277-2301 to order by PHONE. Certificates can also be requested by MAIL with a complete application, photocopy of valid picture ID of the person requesting the certificate and correct fees.  Requests will be processed the same day they are received – please allow mailing time.
