Vital Records (Birth & Death Certificates)
The Tooele County Vital Records Office provides lifetime storage, preservation and maintenance of birth and death records, registration and screening of records for accuracy, cross-indexing birth and death records to prevent fraud, monitoring causes of death for those which may need the attention of the Medical Examiner, issuance of burial and disinterment permits and continual efforts to ensure the confidentiality of vital records.

Certificates can be obtained IN PERSON Monday through Thursday from 8:00 a.m. – 6:00 p.m., Friday 8:00 a.m. – noon, closed weekends, and holidays. The Vital Records Office is located in the Tooele County Health Department Building located at 151 North Main Street in Tooele, Utah in Suite 110. Phone (435) 277- 2301, Fax (435) 277-2304.Staff are available 24/7 for emergencies. Picture ID is required for obtaining birth & death certificates. You must bring a Picture ID with you when you apply for a birth or death certificate.
Certificates can be requested by Mail, applications must include a photocopy of applicants Picture ID. Applications received by mail which do not have a photocopy of Picture ID will be returned.
