Tooele County Health Department provides certified copies of birth certificates for births that occurred in Utah from 1931 to the present and death certificates that occurred in Utah after July 2006 (all other dates available at the State Office – see below how to order online). For marriage records visit the county clerk office where the marriage occurred Tooele County Clerk’s Office website, phone (435)843-3140. Monday through Thursday from 8:00 a.m. – 6:00 p.m., Friday 8:00 a.m. – noon, closed weekends, and holidays. All in-person orders require an application, valid identification and correct fees. Most requests can be completed the same day. Our office is located at 151 North Main Street, Tooele, Utah, STE 100.

BY MAIL – Certificates can be requested by MAIL with a complete application, photocopy of valid picture ID of the person requesting the certificate and correct fees. Requests will be processed the same day they are received – please allow mailing time. ONLINE- You can order birth and death certificates ONLINE from the State Office of Vital Records’ SILVER website (allow 10 to 14 days processing and mailing time)
