Health Program Representative (HPR) / Medicaid
Once you have become approved for Medicaid through the Department of Workforce Services (see below for contact information ), you will need to choose your Primary Care Provider.
The Medicaid Health Program Representative at the Tooele County Health Department can help you with this process. Please phone (435) 277-2320 and ask to speak with the Medicaid Health Program Representative (HPR).
Health Program Representatives (HPRs) will help you understand your Medicaid benefits. HPRs provide education so you can learn about your Medicaid benefits and provider options.
If you live in Utah, Salt Lake, Davis, Weber, Box Elder, Cache, Iron, Morgan, Rich, Summit, Tooele, Wasatch, or Washington Counties, you must choose a health plan for your medical care.
If you live in any other county in Utah, you may have a choice of selecting a health plan or using the Fee for Service Network. An HPR will help you choose a health plan or network that will work best for you.
HPRs are NOT eligibility workers and cannot help with eligibility questions or concerns. If you have questions about your eligibility, please call the Department of Workforce Services (DWS) at 1-866-435-7414.

- UPP Program: UPP (Utah’s Premium Partnership for Health Insurance) helps make health insurance more affordable for families and individuals. UPP (pronounced ‘up’) helps you pay your monthly health insurance premiums through your employer’s health insurance plan or COBRA coverage.
