Immunizations for Jr. High Students

Dear Parents of 6th grade students,

Congratulations on your child’s advancement to Junior High School! As you know students going into Junior High must receive some vaccines to assure they will be safe from transmitting or receiving any preventable diseases. The vaccines required for those starting junior high are:

  • Two documented Varicella (Chicken Pox) vaccines
  • TDAP (Tetanus, Diptheria and Pertussis)
  • Menactra (Meningicoccemia)
  • HPV (Human Papilloma Virus) recommended but not required

At the Tooele County Health Department we are able to give all these vaccines and any others that your child may need. We are a walk in clinic with no appointment required. We are open Monday through Thursday from 9 A.M. to 5 P.M. We accept most insurance and receive payment only for the cost of the vaccines. We never charge a co-payment to you or your insurance. We will gladly work with those with financial constraints or difficulties. Your child’s health and protection is our main goal.
Please bring your child now to any of the above times. By coming in now we can assure you fast and timely service by avoiding the summer rush of students preparing to return to school.

Please call anytime with questions (435)277-2311 or (435)277-2301

Thank you for protecting your children.

Sincerely,

Wayne Lyman, RN, MHA
