Health Program Representative (HPR) / Medicaid
Once you have become approved for Medicaid through the Department of Workforce Services (see below for contact information ), you will need to choose your Primary Care Provider.

If you wish to make a change once you have chosen your Primary Health Care Provider, you must do so by the 20th of the month to have it changed for the following month.

If you are not eligible for Medicaid, you may qualify for Utah’s new Adult Insurance Program for Adults Primary Care Network (PCN) .
- UPP Program: UPP (Utah’s Premium Partnership for Health Insurance) helps make health insurance more affordable for families and individuals. UPP (pronounced ‘up’) helps you pay your monthly health insurance premiums through your employer’s health insurance plan or COBRA coverage.
- CHIP: CHIP(Children’s Health Insurance Program) provides low-cost health coverage to children in families that earn too much money to qualify for Medicaid. In some states, CHIP covers parents and pregnant women. Each state offers CHIP coverage, and works closely with its state Medicaid program.
